I understand that this request applies only to information covered by the Fair Credit Reporting Act Affiliate Sharing provisions* and includes information that may have been obtained about me to determine eligibility for insurance under my current policies, such as my driving record, insurance score and credit history. I also understand that notwithstanding my opt-out request, Erie may share other types of information about me with its affiliates, such as my contact information, policy coverages and payment history.
